To apply for a flight, please fill out the form below with details about your child’s medical condition and the dates travel is needed. We will review your request and contact you within 24 hours (or sooner for urgent cases).

If you do not have a Letter of Medical Necessity ( LOMN ),
please upload a recent medical record instead. The LOMN can be submitted later.
We primarily serve medically fragile children (and some young adults) who need to travel long distances for life-saving medical care and cannot fly commercially due to their condition.
No. All AeroAngel flights are 100% free for families, thanks to donations and volunteer pilots.
We coordinate flights across all 50 states and can access both major and small regional airports to minimize travel time for families.
You can donate, sponsor a flight, volunteer your time, or spread awareness to help us provide more life-saving flights.
We recommend at least three weeks’ notice, but we do our best to accommodate urgent requests for critical medical needs.
